Key Elements for a Texas Evaluation Request Letter

  • Date: The date the letter is sent (essential for tracking timelines).
     

  • Recipient: Name of the principal or District Special Education Director. 
     

  • Student Information: Full name, birthdate, grade, and campus.
     

  • Specific Concerns: Detailed examples of academic, behavioral, or functional difficulties. (i.e., please try to be as specific as you can and include any private diagnoses, medical needs, outside therapies) You may also want to look over the 13 eligibility categories in the Parent’s Guide to the ARD Process booklet and think about which disability category best matches the concerns you’re seeing in your child.
     

  • Formal Request: A clear statement requesting a "Full Individual and Initial Evaluation" (FIIE).
     

  • Signature: Parent/Guardian name and contact information.

If your child is not yet enrolled in school, start by finding out which campus you are zoned to. Once you know the school, call the front office or check the school’s website to locate the email addresses for the principal and assistant principal. Send your request by email so there is written documentation of your communication. Remember: if it isn’t in writing, it didn’t happen.
